📍 Location: Willowbrook, IL

📧 You will ONLY be contacted by email if items are available for your family.

No items are guaranteed, reserved, or held. Requests are fulfilled based on available donations and inventory at the time our Hub Angels shop.

If you are unable to pick up your items for ANY reason, your items will be offered to another family in need. Due to the high demand for assistance, we are unable to hold items beyond the scheduled pickup time.

By submitting this request, you acknowledge and understand that The BCA Hub cannot guarantee the availability of any requested items and that missed pickups may affect eligibility for future shopping events.

🚫 IMPORTANT NOTICE – PLEASE READ BEFORE SUBMITTING

Do not provide false, incomplete, or misleading information, including names, addresses, email addresses, phone numbers, household information, or children's information.

Occasionally, The BCA Hub may be able to deliver items directly to families experiencing emergencies or special circumstances. Providing incorrect contact information may prevent us from reaching you or delivering approved items.

Any applicant found to have intentionally provided false information will be immediately disqualified from all BCA Hub programs and services for one (1) year.

By submitting this form, you certify that all information provided is accurate and truthful to the best of your knowledge.

The BCA Hub Youth & Resource Center is proud to support moms, dads, legal guardians, expectant mothers, and families with children age 14 and younger who are facing emergencies, financial hardship, or recovering from disasters.

🛒 How It Works

🍼 Monthly Hub Angels Shopping Program Guidelines

Our Hub Angels shop for families once per month, typically at the end of each month.

(No shopping event is held in December 2026.)

Families must complete a new request form for each shopping event.

Please Note:

• Items are donated and distributed based on availability.

• Requests are welcome, but no items can be guaranteed.

• Items are provided on a first-come, first-served basis.

• Due to the high volume of requests, we do not respond to emails, messages, phone calls, or text messages asking if items are available.

📞 You will ONLY be contacted if items are available for your family.

If selected, you will receive a text message, email, or phone call with your pickup date, time, and location.

If you are unable to attend your scheduled pickup, your items may be offered to another family in need. Repeated no-shows may affect eligibility for future shopping events.

✅ Proof of Children Required (Fairness Policy)

To ensure fairness and proper distribution of resources, documentation is required for each child listed on this form.

Please upload ONE of the following:

• Medical insurance card showing the child(ren)'s name(s)

• Birth certificate(s) listing you as the parent

• Legal guardianship paperwork

• Letter from a social worker, case manager, foster agency, or another official agency confirming guardianship or placement

🚫 Do not list or claim children who are not legally, biologically, or officially placed in your care.

Submitting false information will result in immediate disqualification from all BCA Hub programs and services for one (1) year.

Please bring your Mom & Me Card to pickup.

If you do not yet have a Mom & Me Card, one will be created during your first approved pickup.

💙 A Note About Our Program

The BCA Hub is a 100% volunteer-powered, self-funded 501(c)(3) nonprofit organization. Our ability to provide items depends entirely on community donations and available inventory.

While we do our best to help every family, submitting a request does not guarantee that items will be available.

We are grateful for the opportunity to support families throughout our community and appreciate your patience, understanding, and cooperation as we work to help as many families as possible.
