THANKSGIVING FOOD BASKET

We work hard to lend a “hand up” to those less fortunate in our community year-round. However we believe all Holidays are a special time, and we are asking for your help to make this Thanksgiving and Christmas season merry for our neighbors in need.

Help us feed more families this holiday season.

We cannot do this alone. 100% of your donations stays right here in our community and goes directly to our programs. We are challenging everyone to give a little this holiday to help provide a warm meal to those in need.

The food donations that we are in need of are:
– Boxed stuffing, mashed potatoes, and noodles
– Canned vegetables
– Canned fruit
– Canned yams
– Canned Soups or Broth
– Small Turkeys or Whole Chickens

We truly appreciate food donations. However, if you do not have the time to shop then through our buying connections, we can make your monetary donations go even further. A $30 donation provides a Thanksgiving & Christmas food basket valued at $90.

Ways to donate:
1. Stop by at any of the BCA Hubs events
2. Email us to set up an appointment to come into the office.
3. Donate right here on our website.
4. Your tax deductible donation helps us continue our mission to help our neighbors in need.
5. Hunger is a 365-day issue any help you can provide is always welcomed and appreciated by our community.

100% of all Donations goes back to the kids & their families and residents in our neighborhood.

THE DEADLINE TO COMPLETE THE 2019 Thanksgiving ASSISTANCE APPLICATIONS IS November 1, 2019

The BCA Hub Youth and Resource Center is busy preparing for its annual Thanksgiving Basket Program which provides under-resourced families with a basket filled with all the ingredients to prepare a traditional Thanksgiving dinner. These baskets are donated by generous members of our community.  If you would like your family to be enrolled in this program, please submit the application online no later than November 1, 2019.

 You must submit required documents to participate.

After submitting your application you will be notified by email ONLY if you are missing or need to update any or all documents. Please check your email including your spam and/or junk folders.

Your basket will be distributed on November 21, 2019, no exceptions will be made.

• Please complete all information below.

• This application is for food only and will be provided by the community for the community via The BCA Angels. If you need toys, please check back in November for our The BCA Angels Toy Drive·

Your application must be complete and correct. In order to enter application into the BCA Angels database system you will need to complete the application and be able to Volunteer at least 2 hours at any upcoming BCA Hubs Youth and Resource Events.

Applications received without this information will be denied!

· List everyone who lives with you full-time, not occasionally.

· Application must be received by November 1st at 5:00 p.m.

IF YOU RECEIVE FOOD STAMPS OR A MEDICAL CARD If you receive food stamps: attach a copy of the food stamp verification letter showing the amount of SNAP benefits you receive including the names of those included in the food stamp case. · If you receive a medical card: attach a copy of the card(s) for each person with this application. Please drop your documents off at The BCA office.

IF YOU DO NOT RECEIVE FOOD STAMPS OR MEDICAL CARD Submit proof of income for at least the last 30 days (pay stubs, employee generated payroll report from employer, etc.).

**The BCA WILL NOT call or mail back applications if there is missing information. You will be denied. It is your responsibility to make sure all required information is submitted with your application.

** Complete only ONE application. All applications are entered into The BCA database. Applications submitted at more than one agency will NOT be accepted and may cause your family to be denied by everyone.

**ALL INFORMATION PROVIDED TO THE BCA IS CONFIDENTIAL AND WILL NOT BE SHARED WITH ANYONE.

SHOULD YOUR APPLICATION BE ACCEPTED, YOU WILL THEN BE ASSIGNED A BCA ANGELS CASE NUMBER.

DISTRIBUTION: THURSDAY, NOVEMBER 21, 2019 (YOU WILL BE ASSIGNED A PICKUP TIME)

YOU WILL NEED TO BRING IN THE FOLLOWING INFORMATION:

• A CURRENT UTILITY BILL SENT TO THE HEAD OF HOUSEHOLD AS A PROOF OF RESIDENCE THAT YOU LIVE IN ONE OF THE COMMUNITIES WE SERVE: BURBANK, JUSTICE, WORTH, CHICAGO RIDGE, OR HICKORY HILLS. IF YOU ARE OUTSIDE OF THESE COMMUNITIES PLEASE EMAIL US.

• A FORM OF IDENTIFICATION FOR EACH PERSON IN THE HOUSEHOLD. EXAMPLES: DRIVER’S LICENSE, PASSPORT, PHOTO ID ETC. BIRTH CERTIFICATES OR MEDICAID HEALTH CARD MAY BE USED FOR IDENTIFICATION OF EACH CHILD IN THE HOUSEHOLD.

• INCOME VERIFICATION – INCOME VERIFICATION DOCUMENTS ARE REQUIRED FOR YOU AND ANY OTHER PERSON IN THE HOUSEHOLD IF WORKING OR RECEIVING ASSISTANCE SUCH AS A PAY STUB, SOCIAL SECURITY BENEFIT, UNEMPLOYMENT ASSISTANCE, ETC.

• PLEASE NOTE THAT THERE IS JUST ONE ORDER PER HOUSEHOLD. IF SEVERAL ADULTS LIVE IN YOUR HOME, PLEASE LIST THEM ALL ON YOUR APPLICATION.

• **  QUALIFYING FAMILIES ARE ALSO INVITED TO PRE-REGISTER FOR CHRISTMAS ASSISTANCE:
